GPs did not have the expertise'
During the inquest, counsel assisting the coroner Kelvin Currie said that despite Sabrina having 17 interactions with health professionals, she had not been properly assessed.

"The GPs did not have the expertise or experience to recognise her condition or to manage her treatment and despite persistent and repeated attempts, the mental health service refused to become more than peripherally engaged," Mr Currie said.

Mr Currie singled out Dr Bernard Westley from the Tristar Medical Group, who had been "dismissive" about the need to refer Sabrina to a psychiatrist.

"He did not see the need to refer her back to the mental health service or to institute any further form of treatment," he said.

"There should be at least some comment about the lack of any real appreciation of the issues affecting Sabrina and her parents by Dr Westley."

The inquest heard Dr Kara Britz from Tristar, who initially prescribed Sabrina a less-than-therapeutic dose of the antidepressant Effexor, was a registrar GP who had failed several exams to become a fully qualified GP.

According to the Sparks Police Department, officers were dispatched at 8:21 p.m. Oct. 18 to the area outside Reed, 1350 Baring Boulevard. Police said an investigation showed Maelynn McLauchlin was crossing the street on foot in front of the school when she was struck by an eastbound vehicle.

McLauchlin was transported to a nearby hospital with serious injuries, police said. She passed away Friday.

The vehicle’s driver, a 17-year-old male, remained on scene and has been cooperating with the investigation, police said. Speed, alcohol and drugs do not appear to be a factor in the collision.

A six-year-old girl taken to hospital with stomach problems died after medical staff missed several chances to save her life, a highly critical independent report has concluded.

The Royal Cornwall Hospitals NHS trust accepted there were a series of failures in the care of Coco Rose Bradford, who died of sepsis.

Coco, who had autism, was not treated for dehydration, the report said, and there was a delay in giving her antibiotics when symptoms suggested she had sepsis.

The girl’s family said it was hard to live with the knowledge that Coco would probably still be alive if relatively straightforward treatments had been administered at the right time.
Coco’s family spotted on 22 July 2017 that she was not eating well and over the next two days she began vomiting and having diarrhoea. In the early hours of 25 July her parents called an ambulance.

She was taken to the emergency department of the Royal Cornwall hospital (known as Treliske) exhibiting symptoms including vomiting, bloody loose stools, and an inability to tolerate fluids.

Coco was sent home but returned to the hospital near Truro the following day and was admitted to the paediatric ward, before being moved to adult intensive care. Her condition worsened and she died on 31 July 2017.

The 10-year-old girl was shot while she and her brother were waiting in the car while their parent was shopping inside the store, an official source told News4 affiliate WAFF.

The sibling reportedly found a gun under the seat of the car and shot the girl in the head, according to police.

The girl was flown to Vanderbilt University Medical Center in Nashville for treatment.

It is unclear at this time if the siblings' parent will face any charges.
